What We Do


CASE MANAGERS

RN Case Managers coordinate care with a team of physicians, nurses, home health aides, spiritual counselors, and volunteers. Case Managers visit their patients routinely for medical assistance, medication, coordination, etc.

COUNSELORS

Counselors provide a variety of services including dietary counseling, bereavement and spiritual support to patients and families.

HOSPICE AIDES

Hospice Aides provide assistance in activities of daily living (including personal hygiene and grooming, dressing, ambulation, bathint, etc.) and light housekeeping. Other services may also be provided upon request.

VOLUNTEERS

Volunteers can provide fellowship, company, and support for patients and respite for caregivers.

SOCIAL WORKERS

Social Workers counsel patients, families, and caregivers. They do their best to assess patients’ needs (including financial need) and coordinate with community resources. Social workers can also assist with funeral arrangements.

Hospice is not a place but a philosophy. Hospice patients are cared for in their home, wherever the home may be: patient’s home, a relative’s, a friend’s, or a nursing home.

What if you change your mind?

Hospice is a choice. A patient may choose to leave at any time. A patient may also be discharged if she or he shows signs of improvement.
